Indications/Uses
COPD: Maintenance treatment of patients w/ COPD, including chronic bronchitis & emphysema, the maintenance treatment of associated dyspnoea & for prevention of exacerbations. Asthma: Add-on maintenance bronchodilator treatment in patients ≥6 yr w/ moderate to severe asthma who remain symptomatic on a combination of inhaled corticosteroid & a long acting β2 agonist & who experienced one or more severe exacerbations in the previous yr.
Dosage/Direction for Use
2 puffs once daily at the same time of day.
Contraindications
Hypersensitivity to atropine, its derivatives eg, ipratropium or oxitropium, or to any component of Spiriva Respimat.
Special Precautions
Not indicated as rescue medication for the relief of acute bronchospasm in COPD or asthma. Immediate hypersensitivity reactions may occur after administration. Not to be used for the initial treatment of acute episodes of bronchospasm or relief of acute symptoms. Not to be used as 1st-line treatment of asthma. Patients w/ narrow-angle glaucoma, prostatic hyperplasia & bladder-neck obstruction. May cause inhalation-induced bronchospasm. Avoid contact w/ the eyes. Not to be used more frequently than once daily. Contains benzalkonium chloride 0.0011 mg/actuation which may cause wheezing & breathing difficulties. May impair ability to drive & use machines. Moderate to severe renal impairment (CrCl ≤50 mL/min). Pregnancy & lactation. Childn.
Adverse Reactions
Dehydration; dizziness, insomnia; glaucoma, increased IOP, blurred vision; atrial fibrillation, palpitations, supraventricular tachycardia, tachycardia; cough, epistaxis, pharyngitis, dysphonia, bronchospasm, laryngitis, sinusitis; dry mouth (usually mild), constipation, oropharyngaeal candidiasis, dysphagia, GERD, gingivitis, glossitis, stomatitis, intestinal obstruction including paralytic ileus; rash, pruritus, angioneurotic oedema, urticaria, skin infection & ulcer, dry skin, hypersensitivity (including immediate reactions); joint swelling; urinary retention (usually in men w/ predisposing factors), dysuria, UTI.
